Nella’s at Autumn Lake Healthcare ranked as the second largest nursing home in Randolph County during the first quarter of 2026, data from the Centers for Medicare and Medicaid Services (CMS) shows.

The facility offered 100 beds, which is two more than Randolph County’s average for the quarter. Nella’s cared for an average of 97.5 residents throughout this period.

The CMS gave Nella’s at Autumn Lake Healthcare an overall rating of 2 out of 5, which is below the statewide average of 2.9 for nursing facilities in West Virginia.

Elkins Rehabilitation & Care Center led as the largest nursing home in Randolph County with 111 beds in use during the quarter. Autumn Lake Healthcare at Crystal Springs held the No. 3 spot, operating 84 beds.

By the first quarter of 2026, Nella’s at Autumn Lake Healthcare had incurred $59,810 in two fines and faced two penalties.

Across Randolph County, West Virginia, there were two for-profit nursing homes in total.

Estimates from the U.S. Census Bureau indicate that the nation’s population of older adults continues to rise while the younger population shrinks. From 2020 through 2024, Americans age 65 and older grew by 13%, as those under 18 decreased by 1.7%. The U.S. older adult population has expanded significantly over the past century, up from 4.9 million in 1920 to more than 61 million by 2024.

A 2021 brief found that nearly half of Americans in the older age group – roughly 28 million people – could eventually need long-term services and supports. Projections also show that over one-third may someday require nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
